  • Your doctor has certified you to be homebound, which means you have difficulties leaving your home without assistance, such as with a cane, wheelchair, special transportation, or the assistance of another person.

  • Part of the certification for eligibility includes documentation from your doctor or other health care professionals that they have had a face-to-face meeting with you within a specified time frame and that the encounter was in regard to the reason you are requiring home health care services.
